<p><strong>Description</strong></p> <p><strong>Supervisor Care Management</strong></p> <p>Under the direction of the Manager, Supervisor, Care Management is responsible for the overall daily operations of the Care Management department for which they oversee. This position will oversee Case Managers and Care Management Coordinator support staff to assure the proper case management of our patients through the continuum of care. The Supervisor, Care Management ensures the Case managers advocate for and assist the patient in the achievement of optimal health, access to care, and appropriately utilizing resources in a manner consistent with the mission vision and core values of Covenant Health.</p> <p>Providence caregivers are not simply valued – they’re invaluable.
